Policies


APPOINTMENTS

Visits are by appointment only.  If you need to cancel or reschedule an appointment, please notify us as early as possible.  For the optimal care, please choose a single provider for all elective appointments.  It may be necessary to see another provider for urgent care or during vacations.

URGENT CARE

We reserve time daily for urgent care needs, but call early in the day to obtain same day care.

REFILLS

For routine medication refills, please ask your pharmacy to send us a refill request.  This helps reduce medication and dose errors.  New medications, controlled substances and dosing changes require an office visit at the time the prescription is issued.  An office visit is required for refills on patients not seen within the last year.

LAB RESULTS

We will contact you by mail if your laboratory tests/Xrays are normal.  Abnormal results will be transmitted by a phone call.   Always contact the office if you do not hear from us.

AFTER HOURS CARE

A physician is available for telephone consultation after office hours for urgencies and emergencies by calling 934-5140.  If this service is used for issues that could have been reasonably handled during normal office hours, an additional fee not covered by insurance may be assessed.

PAYMENT

Payment for services are the responsibility of the patient.  Copays and current insurance information should be rendered at the time of service.  Discounts are available for uninsured patients upon request.  We are happy to adjust fees for those experiencing financial difficulties.
